About the Role

As a Support Coordinator, you will work closely with NDIS participants, families, carers, and service providers to help participants understand and implement their NDIS plans and build their capacity to achieve meaningful outcomes.

Key Responsibilities

  • Support participants to understand and implement their NDIS plans.
  • Develop strong relationships with participants, families, and stakeholders.
  • Coordinate and connect participants with suitable service providers.
  • Assist participants to build independence and achieve their goals.
  • Monitor participant progress and review outcomes regularly.
  • Maintain accurate case notes and participant records.
  • Advocate for participants and support informed decision-making.
  • Work collaboratively with multidisciplinary teams and community services.

About You

The ideal candidate will be:

âś… Experienced in Support Coordination and the NDIS environment

âś… Skilled in working with clients with diverse disabilities, mental health conditions, psychosocial needs, and complex support requirements

âś… Empathetic, professional, and solutions-focused

âś… Able to manage competing priorities and work independently

âś… Strong in communication, relationship building, and problem-solving

Essential Requirements

  • Experience as a Support Coordinator or in a similar NDIS role.
  • Strong understanding of the NDIS and relevant frameworks.
  • Excellent written and verbal communication skills.
  • Current NDIS Worker Screening Check.
  • Current Working with Children Check (if applicable).
  • Current Driver’s Licence and reliable vehicle.
  • National Police Clearance.
  • Full Australian working rights.
